    The form below should be used if you would like to submit an appeal application for your business location if the waste generation level qualifies parcel for lower category and/or materials recycled at location total 50% of total solid waste generated. Any other reason for appeal or if you have questions about the Solid Waste Management Charge call 763-324-3481.

  4. Property Owner Information

    Enter information about the property owner and property.

  5. (Found on property tax statement, example: 11-22-33-44-0055. Use commas to separate more than one property ID #.)

  6. Documentation

    Information about the garbage and/or recycling service(s) at this property must be submitted in order for application to be complete.

  7. Provide one of the following for each garbage and/or recycling hauler or individual that removes garbage and/or recycling from this parcel:

    1) A signed letter from the garbage hauler/individual stating the volume of garbage and/or recycling collected from the parcel and the frequency of pick-up in the past 12 month, often called a "Summary of Service". DO NOT specify tonnage unless the hauler charges based on weight. or 2) Copies of your garbage and/or bills from the past 12 months, showing the size of the container(s) and the frequency of pick-up. or 3) A signed letter from company/organization representative stating the volume of garbage and/or recycling collected from the parcel and the frequency of pick-up in the past 12 month. *** For recycling service, please indicate the types of materials recycled.

  8. Please scan your invoices, weight tickets, or other documentation into a single pdf and upload that file with this form.

  9. Documentation

    Check this box if you need more time to gather supporting documentation.

  10. "I hereby declare that the information and documentation provided in this Solid Waste Management Charge appeal application is correct and complete to the best of my knowledge, and that I am authorized to submit this information on behalf of the property owner. Anoka County has my permission to verify any information submitted as part of this application."
