How do I complete a Chemical Health (Rule 25) Assessment?

If you have medical insurance, check with your insurance provider to see if they cover a chemical health assessment. If they do not, or you do not have health insurance, and cannot afford health care services, you may be eligible for Rule 25 funding. Download a Chemical Health Assessment (Rule 25) application form. You may also contact your probation officer, or call 763-324-1270 for further assistance.
