Independent Community Living Services

The goal of Independent Community Living Services is to promote health and home safety in a community setting. Independent community living with the aid of support services begins with a Long-Term Care Consultation.  The Long-Term Care Consultation (LTCC) is conducted by a team who helps Anoka County residents and their families determine if independent community living is the appropriate choice to meet their needs. Community Health Department Programs for The Elderly

Older adults who qualify may benefit from Independent Community Living Services such as the Elderly Waiver (EW) or Alternative Care (AC) program.

Independent Community Living Services may include:

  • home health aide
  • home delivered meals
  • family caregiver support and training
  • adult day care
  • transportation
  • personal care assistants,
  • medical supplies and equipment
  • companion services
  • Assisted Living services (EW only) or foster care

    • Assisted living can be an apartment, room or companion suite with available services such as housekeeping, laundry, personal cares, activities and meals. It is designed for those who need extra help in their day-to-day lives but who do not require nursing home care. Some facilities offer memory care, which provides specialized staff and programming for those living with dementia or Alzheimer’s disease.

Programs for Disabled Individuals

The Community Health Department manages home and community-based care programs that are an alternative to nursing home and hospital care for qualified disabled and chronically ill children and adults.

  • The Community Alternatives for Disabled Individuals (CADI) Program provides an alternative to nursing home care for eligible disabled children and adults under age 65.

  • The Community Alternative Care (CAC) Program provides an alternative to long-term hospital care for chronically ill and medically fragile adults under age 65 and children.
