Performing Consultations

MnCHOICES Assessments are completed by certified assessors who are either county public health nurses or social workers.  The certified assessor will visit with you in your home or in a long-term care facility to help you identify services you might need or want. They will help you plan those services. The MnCHOICES visit is free of charge.


The Certified Assessor will make recommendations for services and help you:
  • Stay in your own home or apartment
  • Move from a long-term care facility
  • Move to foster care or housing with services
  • Understand the choices available to you
  • Develop a plan to meet your needs
  • Access programs that help pay for services if you are eligible

Information & Assistance

Keep in mind that when requested, the assessment team provides:
  • information and education about long-term care options
  • an in-person visit to assess needs and help plan services
  • information about public programs that can help pay for services
  • transition assistance for people who want to return to the community after admission to a hospital or nursing home


There is no charge for an initial assessment and support planning visit, whether provided in the community, to avoid a move to a nursing home, or as a transition to return to the community.
