Death Record Amendment

A Death Record Amendment is any change made to items appearing on a certificate (except for medical information) either after a certificate is issued or more than one year after the death, whichever occurs first.

Requests within five years after the death can be processed at the Vitals Statistics Office in the Anoka County Government Center. Requests made more than five years after death must be sent to the State of Minnesota's Office of Vital Records. Print the Death Record Amendment application (PDF).

In order to obtain a certified death record, you must show that you have a tangible interest (PDF) in the record.
